Description: The final boss of the Codex and the Shroud, this powerful - and sentient - major artifact has been manipulating events since practically the beginning of the game - it is all but stated to be behind the Devil invasion of Stormreach, and by extension a fair amount of the game's events.

Following Arraetrikos's first failed attempt to invade Xen'drix in The Shroud, the Codex is destroyed in an experiment gone wrong, and its power spread across the planes in the form of Mysterious Remnants, which transform those that possess them into Monster Champions, and Codex Pages, artifacts which give the bearer power over one of the planes and which have a knack of falling into the wrong hands. The Codex is later reassembled in The Codex and the Shroud, only to be severely damaged in battle. After several more attempts to spread its influence across other worlds, the Codex is captured by the Gatekeepers and its sentience unwritten in Vecna Unleashed (quest) - only to fall into the hands of the evil god Vecna. Stealing the Codex back from Vecna becomes the final objective of Vecna Unleashed.

In PnP lore, the Codex of the Infinite Planes is a powerful artifact that can open portals to any world or plane of existence. In DDO, it offers a lore explanation of how adventures and locations from different worlds are accessible from the world of Eberron.
