I'm curious if this item should be placed in a different category. This item is not worn by the user, instead, it is essentially applied as a cosmetic effect, and the profane bonuses are only applied while under it's effects. Input?

Technically the item is "worn" by/on top of your helmet.. If you take your helmet off, it gets removed or vice versa, if you attempt to display your helmet, the effect gets removed. So it seems mostly correct to me... As these intricases are explained in the tips section.
And we otherwise do not have a "cosmetic" hat template, so there's no better option I can think of. If you wish to design a cosmetic hat template, by all means.
Edit: Actaully we can change the "item type" as that current doesn't auto cat anyways.. I'll go ahead and do that, and maybe set it up to auto cat eventually.

What if I 'accidentally' deleted this item? Can I reacquire it? The +1 profane skill bonus just might be the game saver one day.
