Yes, you should worry about file size - it's generally a sign that the image is unoptimized. Only item images with a ton of text like Item:Nightmare, the Fallen Moon will come over the 150K warning as a .png when optimized. If you have Photoshop or Gimp, use the web image exporter, or run it through something like http://www.pngoptimizer.com/
Also, item images should be cropped (not resized) to 420px wide. The item template only likes 420px wide images.
