You have been touched by a shadow, and will die and spawn another shadow, if your Strength is reduced to 0.
This is a debuff from shadows'/shadow creatures' basic attack. If your Strength is reduced to 0 while under this debuff, you will be killed, and a shadow will spawn. At low levels, Ray of Enfeeblement can make this effect deadly if you have a low Strength.
